國家基本公共衛(wèi)生項(xiàng)目考核迫在眉睫，為了更好的迎接考核，樟潭社區(qū)衛(wèi)生服務(wù)中心全體職工上下同欲，積極投入，利用休息時(shí)間走村入戶做好、做實(shí)居民健康檔案隨訪工作，并從5月20日起正式在轄區(qū)內(nèi)啟動(dòng)使用公共衛(wèi)生健康檔案移動(dòng)隨訪系統(tǒng)，確保國家基本公共衛(wèi)生項(xiàng)目考核能順利通過。  “公共衛(wèi)生家庭移動(dòng)隨訪包”攜帶方便，功能齊全，為醫(yī)生和患者都提供了諸多便利。

　　The assessment of the National Basic Public Health Program is imminent. To better prepare for the evaluation, all staff at Zhangtan Community Health Service Center are united in purpose and actively engaged. They utilize their rest time to visit villages and households, ensuring thorough follow-up on residents' health records. Starting from May 20, the mobile public health follow-up system will officially be implemented within the jurisdiction to guarantee a smooth passage of the National Basic Public Health Program assessment.  The "Public Health Home Mobile Follow-up Kit" is portable and feature-rich, providing significant convenience for both doctors and patients.

　　轄區(qū)內(nèi)責(zé)任醫(yī)生上門對慢性病患者進(jìn)行血壓、血糖等檢測，檢測數(shù)據(jù)及隨訪時(shí)照片通過3G網(wǎng)絡(luò)直接連接到健康檔案系統(tǒng)。家庭移動(dòng)隨訪系統(tǒng)包含了高血壓、糖尿病等慢性病的隨訪信息錄入功能和“中醫(yī)體質(zhì)辨識”等功能。

　　The responsible doctors within the jurisdiction conduct home visits to monitor blood pressure, blood sugar levels, and other metrics for patients with chronic diseases. The collected data and follow-up photos are directly transmitted to the health record system via 3G networks. The mobile home follow-up system includes functions for recording follow-up information on chronic conditions such as hypertension and diabetes, as well as features like "Traditional Chinese Medicine Constitution Identification.".

　　責(zé)任醫(yī)生通過33道選擇題對老年人以問答的方式進(jìn)行中醫(yī)體質(zhì)辨識，根據(jù)系統(tǒng)自動(dòng)給出的測評結(jié)果，讓老年人了解自己的個(gè)人體質(zhì)，并在生活起居、運(yùn)動(dòng)飲食調(diào)養(yǎng)等方面進(jìn)行指導(dǎo)與干預(yù)。

　　The responsible physician conducted a traditional Chinese medicine constitution assessment for the elderly through a 33-question multiple-choice quiz in a Q&A format. Based on the automated evaluation results provided by the system, the elderly were informed about their personal constitution and received guidance and interventions in areas such as daily living, exercise, and dietary adjustments.

　　公共衛(wèi)生健康檔案移動(dòng)隨訪系統(tǒng)的啟用使隨訪記錄實(shí)時(shí)更新進(jìn)系統(tǒng)，有效地減少了責(zé)任醫(yī)師數(shù)據(jù)重復(fù)收集、盲目錄入及閉門造車現(xiàn)象，增強(qiáng)了電子檔案的真實(shí)性、及時(shí)性，提高了檔案的利用率，完善了檔案的缺項(xiàng)、漏項(xiàng)、錯(cuò)項(xiàng)，有效地監(jiān)督責(zé)任醫(yī)生下村隨訪的有效時(shí)間，實(shí)現(xiàn)轄區(qū)內(nèi)健康檔案實(shí)時(shí)更新，互聯(lián)、共享。

　　The launch of the mobile follow-up system for public health records enables real-time updates of follow-up records into the system. This effectively reduces redundant data collection, blind directory entry, and isolated decision-making by responsible physicians, enhances the authenticity and timeliness of electronic records, improves record utilization, and addresses missing, omitted, or incorrect entries. It also effectively monitors the effective duration of responsible physicians' village follow-ups, achieving real-time updates of health records within the jurisdiction, ensuring interconnectedness and sharing.
